Aging and Disability Resource Centers (ADRCs)
Aging and Disability Resource Centers (ADRCs) are the first place to go to get accurate, unbiased information on all aspects of life related to aging or living with a disability. ADRCs are friendly, welcoming places where anyone – individuals, concerned families or friends, or professionals working with issues related to aging or disabilities – can go for information tailored to their situation. The ADRC provides information on broad range of programs and services, helps people understand the various long term care options available to them, helps people apply for programs and benefits, and serves as the access point for publicly-funded long term care. These services can be provided at the ADRC, via telephone, or through a home visit, whichever is more convenient to the individual seeking help.

Family Care/IRIS

Family Care is a long-term care program that helps frail elders and adults with disabilities get the services they need to remain in their homes. This comprehensive and flexible program offers services to foster independence and quality of life for members while recognizing the need for interdependence and support. https://www.dhs.wisconsin.gov/familycare/whatisfc.htm

IRIS (Include, Respect, I Self-Direct) is a self-directed program for Wisconsin’s frail elders and adults with disabilities. You have to be eligible for Medicaid to be in the IRIS program.

IRIS is built on the principles of self-determination and self-direction. That means that you will have the freedom to decide how you want to live your life. https://www.dhs.wisconsin.gov/iris/index.htm

Guardianship Support Center

Get information and assistance on issues related to adult guardianship, protective placement, advance directives, and more from the Wisconsin Guardianship Support Center (GSC). Operated by GWAAR, the GSC responds to requests for information through a toll-free helpline or by e-mail. Calls are returned in the order in which they are received. Please note the Guardianship Support Center is required to be a neutral information source.  They are unable to provide legal advice or offer direct representation.  If you would like specific legal advice, please consult an attorney.

They can help with questions like:

• How can I make a power of attorney?
• What are my responsibilities as a guardian?

Wisconsin Independent Living Centers

Independent living centers (ILCs) provide assistance to people with all kinds of disabilities. These agencies were created to promote independence and self-determination for individuals, as well as to offer support and information to families and communities. Services provided by ILCs include: peer support, information and referral, independent living skills training, coordination of personal care and other services, advocacy and community education. Click here for more info
